Mentioning an ex is normal — they were a large part of a life and they'll come up. What matters is the temperature. Neutral references are history. Frequent, emotional, or comparative references are an unfinished thing being processed in front of you.
One mention is not a red flag and asking about it doesn't make you insecure.
Is he telling you about her, or telling her something through you? The second one has heat in it and you can feel the difference.
